Overview of the programme
Some of our health innovation networks have opted to deliver the Chronic Kidney Disease (CKD) workstream. This workstream focuses on improving earlier diagnosis and supporting early medical treatment for people with CKD to slow disease progression and reduce the development of cardiovascular comorbidity.
The aim of this workstream is to ultimately reduce the progression of this condition to end-stage renal disease, including the requirement for dialysis. We are supporting Primary Care Networks (PCNs) to implement consistent national guidance for chronic kidney disease (CKD) and improve diagnosis rates and better treatment outcomes for people with this condition.
Within GP practices, approximately 50% of coded CKD patients reside within areas of high socio-economic deprivation, and therefore a targeted approach will inevitably also reduce health inequalities. We are working with our system partners to ensure that clinical activities are also proportionately focused within areas of higher deprivation and groups of people who are most at risk of developing CVD as a result of CKD.
Case finding is, therefore, a key focus of the programme to detect people with CKD but who are not coded as such based on previous lab results. Where people with CKD have been identified, risk stratification will ensure optimised management according to National Guidelines.
